胰高血糖素:
胰腺的胰岛细胞(α细胞)分泌胰高血糖素,控制肝脏生产葡萄糖和其他燃料以及酮体。
胰高血糖素是在夜间及两餐之间分泌,对维持体内糖和燃料的平衡起重要的作用。它使肝脏分解淀粉或糖原储备,并且帮助肝脏从其他物质制造新的葡萄糖单位和酮体。它还促进脂肪细胞分解脂肪。

不幸的是,糖尿病患者的情况正好相反。进餐时,他们的胰高血糖素水平升高,从而导致餐后血糖水平上升。

GLP – 1(胰高血糖素样肽1),GIP(葡萄糖依赖性促胰岛素多肽)和胰岛淀粉样肽:
GLP – 1(胰高血糖素样肽1),GIP(葡萄糖依赖性促胰岛素多肽)和胰岛淀粉样肽也参与调节进餐时胰岛素。 GLP – 1和GIP是肠促胰岛素分泌激素,肠道释放这两种激素作为信号,使胰岛β细胞增加胰岛素的分泌,同时使α细胞减少胰高血糖素的分泌。 GLP – 1还减慢胃排空的时间,使大脑有胃饱胀感和满足感。

2型糖尿病患者GIP的含量低于正常,β细胞对GLP1的反应有缺陷,这也许可以解释为什么进餐时胰高血糖素不受抑制。
好消息是,人工合成的GLP – 1和胰岛淀粉样肽现在是作为药物,来控制2型糖尿病患者餐后胰高血糖素和餐后血糖。
肾上腺素,皮质醇和生长激素:
肾上腺素,皮质醇,和生长激素也参与保持血糖水平。它们和胰高血糖素(见上文)一起,被称为“应激”或“葡萄糖反向调节激素 ”– 这意味着它们使血糖升高。
肾上腺素从神经末梢和肾上腺释放,并直接作用于肝脏,促进葡萄糖生成(通过糖原分解)。肾上腺素也促进脂肪在肝脏的分解和释放,使它们转换成糖和酮体。

生长激素由脑垂体释放。像皮质醇一样,生长激素反向调节胰岛素对肌肉和脂肪细胞的作用。高水平的生长激素造成胰岛素抵抗。
